Privacy Policy: PK-PD Compass™

Introduction: The PK-PD Compass™ application has been developed as an educational and informational tool to assist and support physicians, licensed healthcare providers, pharmacists and similar professionals who have the appropriate medical judgment, experience and skills to provide the information that is inputted in the PK-PD Compass™. The PK-PD Compass™ is NOT a diagnostic tool and does not provide treatment recommendations — the comparisons for antibiotic dosing regimens provided are for informational purposes and should be independently assessed by you. You should exercise your professional judgment in using this tool and are solely responsible for any patient diagnoses or other information you input into the application.

User Consent to Privacy Policy: Any person accessing, browsing, or otherwise using the application shall be considered a "user." All users are bound by the terms of this privacy policy and the PK-PD Compass™ Terms of Use Agreement (you can find the Terms of Use Agreement under "Settings"). Users consent to the collection, use, and disclosure of personally identifiable user information, anonymized analytics data, de-identified patient information and calculations performed by the app (collectively "information"), to the extent not prohibited by law. If you do not consent to this policy, please do not access or use this application.

Information Provided: Registration information may include de-identified demographic information that is collected in the First Run Experience. Users do not provide social security numbers. ICPD Technologies and/or our authorized contractors may collect information about the device you are using, the time you accessed the app, what browser you use, as well as the city/county for users, tokens and IDs for push notifications. The list of information provided by and gathered about users of the application is not exhaustive as certain device, operating system and geo-location information is automatically gathered with anonymized analytics data when you use the application. Some of this information may expire when you close the application, and some of it may still be accessible afterwards.

Crash reports will automatically be sent to ICPD Technologies' crash analytics platform to aid with making the app better. These reports will include information about what part of the code was executing before the crash, and, potentially, the calculation and demographic data interacted with by the program code at the time of the crash.

Other information that is inputted into the application is de-identified patient demographic information (such as the patient's age (by year only), the region in which the patient lives, weight, and height), and PK-PD target attainment calculations and outcomes. Only de-identified patient demographic information is provided to ICPD Technologies; other information that may be inputted into the application is not provided to us. When using the application, only de-identified patient information should be provided. Please do not input into the application any information that could reasonably identify a patient, such as the patient's name, address, telephone number, email address, social security, medical record, health plan, certificate or license, or any other account numbers, biometric identifiers, photographic images or any other unique identifying number, information, characteristic or code.

The de-identified information will only be accessible to the user and, for some of the information, the operator of the application or its agents and administrators. The operator of the application uses the information to register users, administer the application, provide technical support, to improve user functionality and the educational information that is available to users of the application, and for research.

Opt-out Procedures: A user can opt-out of use of the application by uninstalling the application. The application uses data technology provided by Mixpanel. You may find the Mixpanel Privacy Policy at Mixpanel's website: www.mixpanel.com/privacy. Mixpanel's Privacy Policy may provide additional opt out provisions that may apply to you.

Disclosure and Use of Information: Personal information about users is generally shared only to process a request for a user, to improve the functionality and educational value of the app, or to provide a service to a user, and if we will share personal information with a third party, we will implement measures to have the third party keep this information secure and confidential. We will not receive any personally identifiable information about patients. De-identified information, aggregate information, or other information which does not contain any personally identifiable information may be used by us as part of the functionality of the site, for data analysis, as required for the operation of the application, for enforcement of the terms of use or privacy policy for the application, for quality assurance, research, improvement, marketing and other similar activities, or as permitted or required by law.

Linked Websites or Applications: This application may provide hyperlinks to other websites or applications, and which, when clicked, will take the user to a third party website or application not controlled by us. Any hyperlinked third party websites or applications may collect, use, and disclose information in a manner different than the PK-PD Compass™ application. The application uses data technology provided by Mixpanel. You may find the Mixpanel Privacy Policy at Mixpanel's website: www.mixpanel.com/privacy. We are not responsible for the collection, use, or disclosure of information collected through these websites or applications and expressly disclaim any and all liability related to such collection, use, or disclosure.

Security: We implement reasonable security measures to protect against the loss, misuse and alteration of personally identifiable information, through measures on this application and other administrative and contractual arrangements. This includes the use of SSL encryption for all network communications to third-party services used by the app, such as analytics, as well as for communication's with the app's server components for sensitive information, such as user credentials.

User accounts are secured by passcodes and users are encouraged to select non-trivial passcodes for both their device and for the app itself. We use reasonable measures to protect user information that is stored within our database, we restrict access to user information to those of our representatives who need access to perform their job functions, and we will use reasonable measures to securely dispose of any personally identifiable information that is provided to us by a user. Unfortunately, no system is perfect; therefore, we can make no representations or warranties with regard to the sufficiency of these security measures and cannot guarantee the security of information. Unauthorized entry or use, hardware or software failure, and other factors may compromise the security of user information at any time.

Amendments to Policy: We reserve the right to amend this policy periodically and in our sole discretion. Any amendments made to this policy will be considered effective immediately after the amended policy is posted on the application.
